I noticed you are using the FV Player Pro Video Ads with MP4 format before a YouTube video. Does it work if you disable these video ads?
It works for me on iPad with iOS 12.3.1 on https://foliovision.com/player/demos/roll-custom-video -> “YouTube video with MP4 video in front of it as an ad”

I was having the error on almost every video I played on an iPhone the other day. I did get the error on an iPad with 12.1.4 and an iPhone also on 12.1.4. Now I can’t reproduce the error even with video ads still turn on. The one thing I did since then was turn off and then on again the “Enable HLS.js:” setting. I’m not sure if this actually fixed the issue or not.Would you recommend a different format than .mp4 for the video ads?

HLS.js is not related to the MP4 nor YouTube playback. Perhaps it was some bug in iOS 12.1.4. In my experience sometimes when you play a lot of videos in different tabs on these devices they sometimes stop working reliably and at least the browser needs to be closed ad re-opened.
Perhaps latest iOS improves that.

the video on https://www.ncwlife.com/ncwlife-evening-news-november-11-2019/ worked for me on iPhone with iOS 12.4.3 without issues. The way it works is that the YouTube video requires another tap if it shows after a MP4 video ad.
We are also working on fixing that playlist menu which shouldn’t appear if there is a video ad.
The Video Checker checks the video encoding and YouTube is taking care of the video encoding automatically, so there is no point in checking it. But perhaps it would be good to know if the video is still there for example.
